The Baseline Report and the First Monitoring Report

National Monitoring and Evaluation of the Indigenous Chronic Disease Package - Baseline Report 2009-10 is the Baseline Report for the evaluation of the Indigenous Chronic Disease Package (ICDP or ‘the Package’). The purpose of the report is to form a baseline view of the ICDP against which further findings will be considered and assessed, and to provide information on the first year of implementation (2009-10).

This report contains an introduction and overview of the Indigenous Chronic Disease Package (ICDP) and the evaluation, a discussion of the contextual baseline, the ICDP baseline and individual measure baselines including data analysis relevant to individual measures.
